A Medical Examination is necessary as part of your application for adjustment of status. The purpose of the medical examination is to determine, if you have certain health conditions which may need further follow-up. The information requested is required in order for a proper evaluation to be made of your health status.

The results of your examination would be provided, in a sealed envelope, to an immigration officer and may be shared with health departments and other public health or co-operating medical authorities, as per law.

All Expenses in connection with this examination must be paid by you.

Pl remember to bring the following : 

bulletPassport, if none, could bring border pass (in case from Mexico), or Birth Certificate plus Picture Id or Work Authorization.
bulletAny, or all Immunization Info [In writing, must have Month-Day-Year Specified]
bulletAny "Baby Books" with childhood immunizations noted (mm-dd-yyyy)
bulletAny Special info provided by INS, pertaining to medical examination.
bulletWork Authorization, or "A" number if available.
